Melanocytes are pigment-producing cells that determine the colour of our skin. Production of melanin is dependent on many variables such as hormones, drugs, trauma and UV or sun exposure. It is a natural protective mechanism of the skin when exposed to these elements and factors.

Epidermal (superficial) pigmentation: This type of pigmentation is usually close to the surface of the skin and are induced by sun exposure. It includes solar lentigos, freckles, and cafe-au-lait macules.
Dermal (deep) pigmentation: Dermal pigmentation is usually found in the dermal layers. It includes Hori’s macules and naevus of ota.
Mixed dermal/epidermal pigmentation: This type of pigmentation traverses through both the superficial and deeper layers of the skin. Melasma is an example of this.

Hyperpigmentation

The production of excess melanin causing skin patches that appear darker than the surrounding skin. Age spots, liver spots, freckles, sun spots and melasma are all types of hyperpigmentation.

Excess skin pigmentation or hyper-pigmentation is usually caused by an overwhelming of the melanin-protective system. This leads to excess production and clumping of melanin, and the appearance of brown or darker spots of pigmentation in the skin.

Characterised by the loss of colour to the skin, it is due to the absence of normal amounts of melanin (the chemical that gives skin its colour) caused by disease, injury, burns or other trauma to the skin.

The most common cause of hypopigmentation is damage or trauma to the skin. Burns, infections, pimples, blisters, scrapes and any injuries that result in scarring can all lead to skin discolouration.

Melasma is a chronic skin disorder that results in freckle-like spots and large area of brownish facial patches, it is most prevalent on the face in areas like the forehead, chin, and above the lips. Melasma is sometimes separated into epidermal, dermal, and mixed types, depending on the depth and the colour of the pigment.

What causes Melasma?

The pigmentation is due to the overproduction of melanin by the pigment cells, melanocytes, which is taken up by the keratinocytes (epidermal melanosis) and deposited in the dermis (dermal melanosis, melanophages). There is a genetic predisposition to melasma, with at least one-third of patients reporting other family members to be affected. In most people, melasma is a chronic disorder.

Actinic keratosis or sunspots as they are more commonly referred to as are dark patches on the skin that develop as a result of prolonged exposure to the sun.

Sunspots often develop on exposed areas such as the face, neck, back of hands and forearms. These epidermal spots vary in size and can appear in different ways from smaller brown spots to larger skin-toned patches. Despite being quite harsh in appearance, they are generally not a threat to your body or skin. Even though this can also develop into a form of skin cancer known as squamous cell carcinoma (SCC), the risks of developing this is less likely than other forms of skin cancer. Other types of superficial pigmentations include age spots, freckles and pigmented birthmarks.

What causes sunspots?

It is interesting that the combination of physical factors indicates the likelihood of an individual developing actinic keratosis. Those with fairer skin, blonde or red hair in addition to having lighter eye colours such as blue or green are at higher risk of developing it. Having a history of severe sunburns also increases the risk of developing the spots. It is ideal to note that the development of cancer for this condition is low. However, the risk still exists and as such, it is best to contact a professional if you see changes to the patches.
